چکیده

Abstract Objective The aim of this study was to characterize the tissue involving margin and if information will affect prediction on restaging magnetic resonance imaging (MRI) in low rectal adenocarcinoma (LRC) patients treated with neoadjuvant long-course chemoradiotherapy (LCCRT). Methods In retrospective nonmetastatic LRC (distal <5 cm from anal verge) LCCRT followed by surgery, a radiologist blinded outcome reread MRI documented radial involved tumor, fibrosis, or mucin reaction using T2 high-resolution (HR) diffusion-weighted (DWI). diagnostic performance tumor-involving assessed surgical histopathology as reference. Interobserver agreement between three independent radiologists subset. Results We included 133 (80 males 53 females) mean (range) age 44.7 (21–86) years 82% them had well moderately differentiated adenocarcinoma. Baseline showed T3 (n = 58) T4 60) disease 89% patients. pathological positive 21% 28) cases. There moderate fair inter-reader for type CRM (κ 0.471) mr-TRG 0.266), p < 0.05. use both distance criteria margins improved accuracy 39 66% MRI. Conclusions Margin can be characterizing cancer determining type.

Safe distal margin resection in patients with low rectal cancer undergoing neoadjuvant chemoradiation

Background: Colorectal carcinoma is the third most common cause of death in Iran. This study was performed in order to determine the appropriate distal clearance margin (DCM) for resection of rectal cancer in patients who undergo neoadjuvant chemoradiotherapy for sphincter preserving procedure. Materials and Methods: This was a cross-sectional study conducted in Shahid Faghihi Hospital of Shira...

Predictors of positive radial margin status in a population-based cohort of patients with rectal cancer

BACKGROUND Surgical margin status is an important predictor of risk of relapse among patients with rectal cancer. METHODS Patients referred to the British Columbia Cancer Agency for consideration of adjuvant therapy for rectal adenocarcinoma were included. Predictors of margin positivity were determined from uni- and multivariate analysis. RESULTS Among 340 patients, 83% had negative resect...

Neoadjuvant treatment in rectal cancer: do we always need radiotherapy-or can we risk assess locally advanced rectal cancer better?

There is good quality evidence that preoperative radiotherapy reduces local recurrence but there is little impact on overall survival. This is not completely unexpected as radiotherapy is a localised treatment and local control may not prevent systemic failure. Optimal quality-controlled surgery for patients with operable rectal cancer in the trial setting can be associated with local recurrenc...

Hypnosis in Cancer Patients: Can We Do Better?

There is an official history of hypnotism: very often it begins with the Scottish surgeon James Braid (1795-1860) who was one of the first physicians to have used this word in 1843. Instead of using the standard magnetic induction popularized by magnetism/mesmerism1, he proposed his subjects to stare steadily at a shiny object to enter a trance. He tested his method to anesthetize his patients ...

Pain control in ambulatory cancer patients--can we do better?

To evaluate the degree of pain control among ambulatory cancer patients visiting the outpatient clinics of three oncology centers in south Israel, these patients were interviewed using the Brief Pain Inventory translated into Hebrew (BPI-Heb). Patients suffering from pain at least three times a week or reporting taking daily analgesics during the last two weeks were enrolled. Non-Hebrew speaker...
